Application

Please complete the application online and print.  Send the completed application along with a $200 nonrefundable deposit.  There will be no refunds awarded after May 1st.  Make deposit checks payable to Stick to Stick Field Hockey Camp.  Final payment is due upon arrival in cash or by check. Upon receipt of the application and deposit, campers will receive a confirmation letter  and travel form. Campers must have all forms completed and submitted during check in.  Campers will not be able to participate in camp without all of their forms completed.  Mail completed application to:
